Jody Maluski, DPT, CSCS - Clinic Director
Jody, originally from West Fargo, North Dakota, received her Bachelor's degree in Biology from College of Great Falls, MT. After working for a year in her field, she returned to University Wisconsin-LaCrosse were she received her BS in Physical Therapy. Jody has her certifications in Graston and Rock Tape and is a Certified Strength and Conditioning Specialist. She is also an APTA credentialed clinical instructor. Jody's experience has primarily been in outpatient orthopedics with over 25 years of experience in treating post-surgical patients, sports rehab, and general orthopedics always in the mix.
Outside of work, Jody enjoys sporting events all the way from High School to professional, golf, fitness and spending time with her family.
Erika Rocabado, PTA
Erika Rocabado is a Physical Therapy Assistant in Chandler and attained her Associate's degree as a Physical Therapy Assistant from Gateway Community College. She performed her last student rotation with Rise Rehab (then Pinnacle Physical Therapy) in 2008 and has been a part of the team ever since. Erika is certified in LSVT/BIG, a rehabilitation therapy for patients with Parkinson's Disease, and has advanced training in shoulder impairments and treatments. She also excels in hands-on treatment. Her passion for physical therapy began while she was a member of the athletic training program at Dobson High School in Mesa, AZ, and increased when she was a patient recovering from back surgery after high school. She is fluent in Spanish and enjoys traveling, watching movies, and being with family and friends.
Bhoomi Patel, born and raised in western India, received her Bachelor's in physical therapy with honors from Gujarat University, India. After graduating, she came to the U.S to pursue her masters in exercise science and cardiac rehabilitation from the City University of New York (CUNY), Brooklyn. Besides Arizona, she also holds a PT license from the state of NY. She is certified LSVT/BIG program for Parkinson's disease and has received advanced training in vestibular therapy, joint replacements, orthopedic injuries and surgeries. Bhoomi's primary experience has been in orthopedic outpatient services and she is comfortable with all age groups. She is fluent in English, Hindi and Gujarati, and is passionate about travelling to new places, art and spending time with family and friends.
Kim Lee, PT - Director in Training
Kim comes to Rise Rehab with 25 years of physical therapy experience. She is from Evansville, Indiana, having attended Indiana University for pre-physical therapy course work and graduating from university of Evansville in 1988 with a Bachelor's degree in Physical Therapy. Kim is ASTYM certified, and has advanced training and extensive experience in shoulder and lumbar rehab. Kim's experience also includes treatment of high school and collegiate athletes, with experience in post-surgical rehab and sports rehabilitation. Early in her career, Kim worked with many neurological conditions, including Cerebral Vascular Accident (CVA) and spinal cord injuries, prior to specializing in orthopedics. When Kim is not working, she enjoys spending time with her husband, Todd, and stays busy with their three boys.
Kayla Botts, AT
Kayla Botts graduated from California State University, Bakersfield in 2008 with a Bachelor's degree in Kinesiology. During her time at CSUB, she worked two years as an intern with the Bakersfield Condors ice hockey team. She then attended Northern Arizona University to study Athletic Training. Kayla worked one season with Big Sky Champions, Women's Soccer and completed a year-long clinic internship at Canyon del Oro High School in Tucson, AZ. After graduating from NAU in 2011, Kayla worked as an Assistant Athletic Trainer at Canyon del Oro from 2011 to 2012. In 2012, Kayla began working as the Head Athletic Trainer at Maricopa High School and has been with Rise Rehab since 2014.
Chelsie Wise is from Louisville, Kentucky and attained her Physical Therapist Assistant degree from Jefferson Community and Technical College. Before becoming a PTA, she was a nanny who specialized in caring for children with special needs. She has always had a passion for helping people and giving back to the community and is very excited to be joining the Rise Rehab family in Maricopa!
Outside of work, Chelsie enjoys cooking, painting/sculpting, sewing, reading, working on cars, and having wild dance parties with her husband and 5 year old daughter.
Caitlin Hausman, PTA
Caitlin Hausman attained her degree for Physical Therapist Assistant from Pima Medical Institute. Prior to becoming a PTA Caitlin worked as a massage therapist and physical therapist technician which is where she realized she wanted to further her career in physical therapy. What she enjoys most about being a PTA is meeting new people on a daily basis while positively impacting their quality of life. When Caitlin is not working she enjoys keeping her life active with hiking, snowboarding, scuba diving and fitness classes.
Jared Egan, PT
I grew up in Detroit, Michigan with 4 older brothers playing soccer, football, basketball, lacrosse, and MMA. I received my Bachelor of Science in Exercise Science at Brigham Young University in 2013. I then moved to Arizona to attend A.T. Still University - Arizona School of Health Sciences, graduating in 2017 with a Doctorate in Physical Therapy degree. Most of my experience and interest are in orthopedics and neuro rehab. I am engaged to be married to my best friend, and we will have 4 kids together and our puppy, Luke, to keep us busy for the next 16+ years. I enjoy spending time with my family, eating good food, watching movies, camping, and going to the gym with my "swolemate".
Jennifer Halvorsen, PTA - Clinic Director
Jennifer attained her Physical Therapist Assistant degree from St. Catherine University in Saint Paul, MN. Prior to PTA school, she received her Bachelor of Arts degree in Health & Exercise Science from Gustavus Adolpus College - St. Peter, MN. Shortly after graduation, she became an ACSM Certified Personal Trainer, ZUMBA instructor, and health coach/fitness specialist. While attending St. Catherine University, Jennifer received the 2013 American Physical Therapy Association (APTA) Mary McMillian Scholarship, awarded to recognize superior scholastic ability and potential for future contribution to the physical therapy profession. Jennifer has experience in occupational health and orthopedic settings in Florence, Coolidge, and Phoenix areas. She is a member of the Orthopedic Section and Arizona State Chapter of the American Physical Therapy Association (APTA). Outside of work, Jennifer enjoys fitness, reading, dancing, playing with her Labrador retriever, and the Minnesota Vikings.
Garrett Johnson, PT, DPT
Garrett is from Missoula, MT and graduated with his Doctor of Physical Therapy from the University of Montana in May 2016, where he also received his Bachelor of Arts in History and his Graduate Certificate in Public Health. He also attended the University of Alaska - Anchorage for pre-physical therapy coursework. While pursuing his DPT at UM, he was also a member of the Montana Department of Health and Human Services, Women's and Men's Health Section, Information and Education Committee. Garrett prefers to treat using exercise, manual therapy techniques and modalities including electrical stimulation, while focusing on improving function and quality of life for his patients. When away from the clinic, Garrett enjoys sports of all kinds, hiking/backpacking/camping, reading, movies and spending time with his wife and two children.
Joshua Hamilton, PT, MPT, DPT - Clinic Director
A native of Wyoming, Josh graduated from Tongue River High School in Dayton, Wyoming. He enjoyed playing football and had plans of possibly pursuing a college career, but during his senior year he sustained a career ending knee injury. Through his ACL reconstruction and subsequent rehabilitation, he became interested in physical therapy and decided to pursue a career in outpatient therapy. He immediately enrolled in college after graduation and spent two years at Maranatha Baptist Bible College in Wisconsin, where he completed many of his prerequisite classes. He then transferred to the University of North Dakota and was accepted into their physical therapy program. Josh received his Bachelor's, Master's, and Doctorate degree in Physical Therapy from the University of North Dakota. He is also an APTA credentialed clinical instructor. Josh has worked in the areas of outpatient orthopedics and sports rehabilitation for the past nine. Josh resides in San Tan Valley with his wife and their four children, daughters, Adlea and Avery; and sons, Landen and Layton.
Kyle Black, PT
My name is Kyle Black, and I grew up in Glendale, AZ. I am the oldest of 4 kids. I grew up playing and loving sports! I played football and baseball at Moon Valley High School in Glendale, and my introduction to physical therapy came from my sports injuries I sustained in high school sports. I graduated from Brigham Young University in Provo, UT with a degree in Exercise Science and am a huge fan of BYU football and basketball! I met my wife, Cali at BYU and we have been married for 4 years. We are expecting our first baby, a girl, at the end of November and we are very excited! I graduated Physical Therapy School at AT Still University in August and I am very excited to get my career started. My passion is orthopedic and sports physical therapy, and because I went to physical therapy as an athlete, I love helping other injured athletes return to the sports that they love to play! I am looking forward to getting my career started here at Rise Rehab!
Blain Packard, PTA
Blain was born and raised in a small town in Kansas...which is one of the reasons he feels at home in Florence and Coolidge. After many years as a physical therapy technician and several knee surgeries followed by intense physical therapy, he went back to school to become a Physical Therapy Assistant. Blain graduated from Gateway Community College in Phoenix, Arizona and has been helping people get better ever since. Rock Chalk Jayhawk!!!
Will Humphreys, PT, Owner and CEO
Will was born and raised in El Paso, Texas. It was as a high school senior that he first became introduced to the field of Physical Therapy when he became a patient himself after a critical rock climbing accident that left him with broken arms, and legs. During this time, he decided he wanted to become a Physical Therapist and help provide the same light and hope to others he was given by his Physical Therapist. He has a wonderful wife and four boys. He enjoys creating an environment that promotes family, fun, and freedom for his team and customers.
Nathan Shields, PT, Owner and CEO of Rise Diagnostics
Nathan is a native of Arizona. After graduating from Northern Arizona University's Physical Therapy program in 1999 Nathan began working in the Phoenix area, eventually opening up his first PT clinic in Chandler - Pinnacle Physical Therapy - in 2002. Within a few years he teamed with Will Humphreys to open more clinics in Florence and Maricopa, and over time they developed Rise Rehab. As of June 2016 Nathan started a new venture, Rise Diagnostics, in Alaska!!! When he is not developing new companies in far off places, Nathan and his wife Whitny stay busy with their six boys and little girl.
Michelle Bambenek, Vice President of Operations
Michelle, originally from the Kansas City Missouri area, received her Bachelor degree in Exercise Science from Truman State University in 2003 and became a published author for research she completed during her undergraduate career. Shortly after graduation, she moved to Mesa, Arizona to attend A.T. Still University - Arizona School of Health Sciences, graduating in 2007 with a Doctorate in Physical Therapy degree. Michelle currently resides in the Chandler-Gilbert area with her husband, Scott, and in her free time she enjoys spending time with her husband and friends as well as flower arranging, playing tennis, watching football and spending time in the gym herself.
Stacey Sullivan, Vice President of Public Relations and Marketing
Stacey is our Vice President of Public Relations and Marketing. Stacey started with the company almost 12 years ago as a Physical Therapist Assistant for the Chandler clinic. Shortly after starting, Stacey was given the opportunity to gain experience in marketing for the company to the doctor offices and community. Stacey has realized as much as she loves working in the clinic with our patients, she has the same passion for marketing, building relationships with our MDs, and our community as well as promoting our amazing company! She is originally from Upstate New York, where she obtained her Associates of Applied Science degree in Physical Therapy and worked in the PT field before moving to Arizona in 1999. She has been a member of the Rise team since 2003. Over the years, Stacey has gained experience in geriatrics, orthopedics, post-operative, & sports rehab. She has taken advanced coursework for applied techniques of Manual Therapy-Mobilization of the Cervical and Thoracic Spine and Ribs. Stacey is also a Certified Clinician for ASTYM as well as the LSVT-BIG treatments for Parkinson's Disease & other neurological conditions. Outside of work, Stacey stays busy with her husband Kevin and their 3 children.
Stephanie Bourland - Communicator
Stephanie Bourland was born and raised in the small farm town of Coolidge, Arizona where she currently resides with her wonderful four Daughters. Stephanie started in 2013 as a patient care coordinator and has been with the rise team for over 3 years and counting. She is very family orientated and on her free time Stephanie loves to head to the White Mountains in Arizona for camping trips with her family, Stephanie enjoys spending her free time traveling and watching her daughters grow and enjoy their many ventures.
